How Scoring Works

Four pillars. One transparent number.

Every decision score is built from 25 points in each dimension — adding up to 100. No black boxes.

Evidence Quality
Randomized controlled trials score highest. Cohort studies, case series, and expert opinion score progressively lower — the way evidence should work.

Guideline Concordance
Does the proposed treatment align with AAOS, AHA, AAP, or other specialty guidelines? Concordance adds points. Off-label or guideline-contradicted approaches subtract them.

Outcome Breadth
Large sample sizes, long follow-up, diverse populations, and patient-reported outcomes all increase confidence. Narrow or short studies score lower.

Access Equity
Is this treatment widely accessible? Cost-effective? Covered by most insurance plans? Treatments that only work for the wealthy score lower on this dimension.

Honest gaps. A condition–treatment pair that is not scored shows no number, never an invented one.
Physician review (next, not built). A way for physicians to review a score and improve the index is planned.
Why it matters for joints. As CMS moves toward mandatory bundled payments for joints, documenting how a decision was made matters more.
